The Agency for Healthcare Research and Quality (AHRQ) mission is to improve the quality, safety, efficiency, and effectiveness of health care for all Americans. The goal of the US Department of Defense Patient Safety Program (PSP) is to avoid medical harm and improve patient safety by focusing on improving systems and communication among health care teams. The DoD PSP is located in the TMA Office of the Chief Medical Officer with oversight of the Patient Safety Center data repository, located at the Armed Forces Institute of Pathology; the Center for Education and Research in Patient Safety (CERPS), located at the Uniformed Services University of Health Sciences (USUHS); Service PSP coordination, and the Healthcare Team Coordination Program (HCTCP). The objectives of the PSP are to improve the coordination of Patient Safety activities across the services, develop an analysis plan for PS data to uncover opportunities for improvement in the MHS; create a culture of trust in reporting medical errors, and increase patient awareness and involvement in PS initiatives.
